Client Terms of Use

I hereby acknowledge and agree to the following policies, procedures, and terms of service:

 

'Farmacy Health Coaches' and 'Health Coaches' refer to Matthew Kelly, Joy Sparks Light, and Zena Kandah.

 

ASSUMPTION OF LIABILITY AND RISK:

  1. Health coaching aims to improve overall health, vitality, and well-being through education, natural foods, non-medicinal nutritional supplements, and lifestyle changes. The Farmacy Health Coaches do not diagnose, treat, or cure diseases, disorders, or conditions.
  2. Matthew Kelly, Joy Sparks Light, and Zena Kandah are not licensed Medical Physicians (MDs / doctors).
  3. As part of the Health Coaching Services, I may be asked to provide information regarding my or my child's physical habits, medical history, moods, energy levels, preferences, lifestyle, and diet. This information enables Farmacy Health Coaches to assess health habits and recommend lifestyle changes to enhance overall health, vitality, and well-being. All information will be held in strict confidence within Farmacy and personally identifiable information will not be disclosed to any third party without prior consent except as required by law.
  4. If Farmacy Health Coaches suspect a disease, disorder, or condition in myself or my child, I will be informed of this suspicion. I acknowledge that this does not constitute a diagnosis, and I am directed to consult a medical professional for further evaluation promptly.
  5. If I request recommendations for dietary changes, lifestyle changes, and/or nutritional supplements to enhance my or my child's body's natural ability to resist or overcome a known disease, disorder, or condition, I am responsible for disclosing all relevant details to the Farmacy Health Coach I am working with. I understand that if I or my child has not consulted with a licensed Physician or Naturopath, I am strongly encouraged to do so. I agree not to alter or discontinue treatments prescribed by licensed healthcare professionals without consulting them first.
  6. In providing Health Coaching Services, the Health Coaches rely on the accuracy and completeness of the information I provide about myself or my child. I understand that any recommendations I choose to follow regarding diet, lifestyle, or nutritional supplements are my sole responsibility. Farmacy Health Coaches are not liable for my or my child's health or safety.
  7. In consideration of participation in Health Coaching Services, I assume all risks, including injury or death, arising from such participation for both myself and my child. I release Farmacy Health Coaches and their representatives from any and all claims, costs, or damages arising from any illness, injury, or death that may result from our participation, regardless of the cause.
  8. I understand that any therapies I undertake with Farmacy Health Coaches for myself or my child are voluntary, and I bear full responsibility for healthcare decisions. Farmacy Health Coaches reserve the right to refer me to other professionals if my or my child's case falls outside the Health Coaches' scope of practice. I assume full responsibility for any loss, injury, or damage that may result from participation for myself or my child. I have read and fully understand this release, waive liability, and I voluntarily agree to its terms.

 

CANCELLATION AND RESCHEDULING POLICY:

  1. I understand that Farmacy Health Coaches require at least 5 full business days' notice to cancel a consultation and receive a refund. At least 3 full business day's notice is required to reschedule. Rescheduling and cancellation can be done with the link in the Calendly email I received when booking.
  2. I understand that not showing up for a Zoom or phone call consultation does not incur any refund or free rebooking of the missed appointment.

 

BILLING POLICY & REFUND POLICY:

  1. I understand that no refund is given if I miss my scheduled appointment. I can rebook an appointment at the regular rate.

 

COMMUNICATION AND CLIENT PROCEDURES:

  1. I understand that if I do not submit all required information (e.g., completed assessment form, lab results) at least 72 hours before my or my child's appointment, Farmacy Health Coaches may not have time to review them.
  2. Follow-up communication via email will be about what was covered during the consultation and the recommendations made to me only. I will book a new appointment when I have new questions that were not covered during our consultation.
  3. Messages and inquiries will be answered within 24-48 business hours during normal business hours (Monday - Friday, 9 am - 4 pm EST).
