1. Email office@farmacy.com with your health history and current symptom list.2. Attach any labs you have had completed in the last 6 months to that email.Please send this as soon as possible so that Matt has time to review all your data. Data sent within 48 hours of consultation may not have time to be reviewed.

Policies, Procedures, Terms and Conditions:

1. Contact office@farmacy.com if you need to cancel or reschedule an appointment. There’s no charge if you cancel before 48 hours of the appointment.
2. Cancelling within the 48 hours before your appointment will incur a refund of only 50%. 
3. If you use another source for recommended functional labs there is a $200 per test lab read/interpretation fee. No lab read fee if you utilize farmacy.com’s labs through Vibrant America on our ‘Functional Labs’ page. No fees for reading labs you provide before your initial appointment. 
4. We are not able to accept insurance for consultations, products or services. 
5. Matthew Kelly does not diagnose or treat any disease, that can only be done legally by medical doctors. He operates by coaching clients on how to balance physical and chemical stressors to promote your bodies own healing and well being.
